To all our valued guests,

As required by the Ontario Provincial Government and Public Health Agencies, CJ’s
Tap & Grill has prepared a Covid-19 Vaccination policy that clearly outlines the
requirements of all our guests and patrons 12 years of age and older. The health and
safety for all continues to be our priority as it has been from the beginning of this
pandemic. With this in mind, we are putting in place the following COVID-19
vaccine requirement policy as per The Government of Ontario’s requirements:

As of September 22, 2021, all guests patrons and visitors who are 12 years and older
will be required to be fully vaccinated with a COVID-19 vaccine series.

• In the case of exemptions CJ’s Tap & Grill will comply with human rights obligations
and accommodate individuals who are legally entitled to an exemption from this vaccine
requirement policy. Unless an official doctors note can be provided accommodation will
include take out/curb side pickup or outdoor/patio dining.

Our goal is to safely serve our community with a great and safe dining experience that
meets and, in some cases, exceeds public-health requirements for infection, prevention,
and control. We have adjusted our protocols along the way, based on shifting conditions
and evolving guidance from governments and public-health authorities.

You our guests and patrons have been crucial in how far we’ve come in the fight against
COVID-19. Your adherence to the requirements along the way has allowed us to open
and operate safely to this point. The concerning rise in the number of COVID-19 cases
due to the Delta variant, however, threatens the progress we have all made in
responding to the pandemic so now we must adhere to this new requirement of vaccine
passport protocol. As we consider the vulnerable that we serve and the presence of
children in our restaurant – many of whom are too young to be vaccinated – we must do
everything we can to protect their health and safety.

Upon entry of the restaurant people will be asked to provide valid documentation of
vaccination status or use the Ontario government’s vaccine verification tool, once it is
available, as the means for demonstrating proof of vaccination. For those who deny
proof we will offer take out or patio dining. No exceptions.

As we work toward the September 22 deadline, all our current health and safety
protocols will remain in place.

It has been an honour to serve this community for over 30 years and we have a
profound respect for your confidentiality and trust. All personal information will continue
to be protected and secured in the most rigorous fashion. We appreciate your continued
support as we adapt our practices to help reduce the spread of COVID-19.

Thank you for doing your part to protect one another and the vulnerable..
